Job Description

The Pediatric Care Team Nurse collaborates with the Pediatric Care Team to coordinate and manage pediatric patient care within Christ Community Health. Duties include but are not limited to vaccine administration, case management, develop patient education materials, etc.

Part Time Pediatric Care Team Nurse Description:

Part Time: 28 hours/week o Tuesdays 8:00am – 5:00pm o Wednesdays 8:00am – 12:00pm o Thursdays 8:00am – 5:00pm o Fridays 8:00am – 5:00pm · Collaborates with Pediatric Care Team: Pediatric Nurse Practitioner, Registered Medical Assistant (MA), and Patient Advocate to coordinate and manage pediatric patient care within community health center. Assists MA with duties during high volume or high acuity to minimize patient wait times. Uses nursing judgment to delegate responsibilities to MA and Facilitator/Patient Advocate, as needed to aid patient schedule flow. · Vaccine administration, knowledgeable in pediatric immunization schedules, and state immunization registries (GRITS and CARES) · Triage: Responds to telephone calls and patient walk-ins · Schedules and conducts Nurse Visits (e.g. blood sugar management, EKG, spirometry, DepoProvera administration, cyanocobalamin administration, therapeutic injections, patient education, immunizations, TST administration and reading, UA, urine culture, microalbumin testing, hCG testing, lead testing, Hbg testing, weight assessment, sutures, ear lavage, hearing screening, vision screening, nebulizer treatment, wound care, Asthma assessments) · Intervention during Office Visits where licensure is required (e.g. IV fluid administration, urinary catherization) · Develops patient education material for pediatric patients and families · Builds effective interdisciplinary relationships · Works with Pediatric Nurse Practitioner and Nurse Manager to develop and establish Case Management and Care Coordination practices to facilitate high quality care throughout the healthcare system · Coordinates outreach events, such as pediatric flu vaccine clinics with local schools 


Opportunities to earn additional certifications and create new or support current programs may be available such as : Wound Care, Ostomy, and Continence Nursing (WOCN), Faith Community Nursing (RN-BC), Advanced Public Health Nursing-Board Certified (APHN-BC), Ambulatory Care Nurse Certification (RN-BC ), Certified Diabetes Educator (CDE), Clinical Nurse Leader (CNL), Accredited Case Manager (ACM), AIDS/HIV Certified Nurse (ACRN or AACRN), Certified Disability Management Specialists (CDMS), Certified Developmental Disabilities Nursing (CDDN) Requirements: · Interest in care coordination and long term care of patients in a Medically Underserved Population (MUP) · Commitment to community health and outpatient care, serving pediatric patients and community health center as Advocate, Collaborator, Consultant, Coordinator, Deliverer of Services, and Educator · Computer skills: knowledge of EMR, Microsoft Windows, Microsoft Office, Google Chrome minimum requirement


Required Education and Experience: 


A diploma in practical nursing, an associate’s degree in nursing, bachelor’s degree in nursing, or the equivalent is required. · A current, valid LPN license or RN license in the state of Georgia is required. · BLS certification is required, PALS certification is favorable. · 2-3 years of previous nursing experience is appreciated. · Community/Public Health experience and GRITS training are desirable
